Sport Psychology Services Agreement

Welcome to my practice.

The document outlines a simplified consent/contract form for mental performance consulting services provided by Andrea Carvalho Dias, focusing on sport psychology and biofeedback. Please read it carefully and email me any questions you might have ([email protected]). When you sign this document, you will sign an agreement between us.

  • Scope of Services: The services include teaching skills for exercise and sport contexts, performance improvement, and evidence-based assessment and intervention, but exclude diagnosis or treatment of mental health disorders and work outside the practitioner's scope of practice.
  • Confidentiality and Exceptions: Client information is kept confidential and securely stored, with exceptions for harm disclosure and legal subpoenas. Clients under 18 have their records accessible to parents or guardians.
  • Session Details and Costs: Mental Training interventions require a minimum of six meetings, with costs outlined for initial evaluations, hourly rates, and discounts for packages. Loaning Biofeedback equipment from my office for home training practices may incur extra costs.

Mental Training Package for Teams

This mental training package is for coaches who are too busy working on the physical aspects of sport performance and have no time to teach the mental game. The package includes the assessment and eight sessions of 45 minutes. The client is the entire team meaning that it is a group training (NOT individual sessions with each member of the team). The sport psychology professional can directly instruct the team (in group sessions) or instruct the coach, who will apply the assessment and implement the mental training program with the team.
